The biggest issue with Fallout 3 is Games for Windows Live (GFWL), which was discontinued by Microsoft. I run Fallout 3 (and all Fallout games) through Mod Organizer as it's my preferred launcher as well and I don't run into any issues with Fallout 3 after installing the required fixes.
